Did you secure the bolt into the weapon?

yes the bolt slides in and is stopped by the top which is wider... i put some tape around the bolt it self to reduce movment from side to side and i taped the top down very well.. then put a piece of foam over it and taped that down very well then covered it in black hockey tape... so its not moving

You only have one layer of foam over the bolt? Not good King-o-sabbi. You'll need at least 2 for ANY pommel, a bolt underneath would probably requires 3. To be safe, you can't feel the bolt through the foam, which I guarantee you can through 1 layer.
Standard method is you make a wrap of foam around your core about 2 inches up onto the core, extend this down to encompass any bolt that would be sticking out. Adhere to your core via DAP or double sided carpet tape (aka DSCT). Put a cap of foam on that wrap of foam, again with DSCT or DAP. Apply another wrap of foam around your first and extend it down to encompass your first cap, with DSCT or DAP. Then add another cap to that foam wrap, with DAP or DSCT. Then cover the works in black tape.
Optionally you can put a cap of foam onto the bare bolt head, THEN do your first wrap around the core, then cap, then wrap, then cap. That' makes dang sure your pommel doesn't fail for "hard spots".

I think he meant he put a foam cap on BEFORE he replaced his pommel, Stubs.

you should prolly cover up anything that is metal with some tape as curtesy (sp) padding to yourself and other fighters.
The way I countereweight my 12-13 oz graphite clubs is using the soldering wire from walmart and duct tape before i put on my pommel.
The way i counterweight my 22-23 oz graphite clubs is soldering wire and lead weights from phil's hobby shop on clinton. but they are more expensive but those clubs dont break so I dont use too many.
some graphite clubs last years...some days. just depends on the core you happen to grab
